  • Employee Transfer E-Form: This form must be completed by an appointing authority for each employee they wish to transfer as governed by one of three RIGL statutes: RIGL 36-4-34.1, RIGL 36-4-34 and RIGL 42-40-3. Prior to completing the form, the signatory should review the Employee Transfer Policy for further clarification as to the procedures for compliance with each of the transfer action types as well as the applicability of this form.
  • Criminal Record Supplemental Questionnaire. Form used to inquire about an applicant's criminal convictions at the initial job interview or any time thereafter.
  • Documentation of Oral Reprimand Form - When issuing discipline to an employee, you must contact your Human Resources Office for assistance with the disciplinary process.
